CMP LIABILITY AFFIDAVIT |
The CMP requires that anyone
who participates in any CMP Club event (non-members as well as
members) have a CMP
Liability Affidavit on file with the
local club.
To get to the CMP Liability Affidavit
form you must go to the CMP's website at www.odcmp.com
On the CMP's Home page click on the tab labeled "FORMS INDEX".
On the Forms Page locate the affidavit under the section
"OTHER FORMS
(Clubs Only)"
titled "AFFIDAVIT AND INSTRUCTIONS".
Click on the link and print off
the form. It is in .pdf format. You will need to have Adobe Acrobat
installed on your computer in order to do this.
Fill out the form as instructed.
It must be signed, dated, and notarized. You can mail the
CMP Liability Affidavit
to the club with your OOS Membership
form, or if you are a non member wishing to shoot as a guest,
just bring it with you the day you come to the match.
